On Your First Visit

To Prepare

It is a great help to us to have your relevant medical history sent to us ahead of time. Please be sure to have any pertinent medical records from your primary care physician faxed to our office in advance. At least tell us your reason for coming. That way we can obtain any referral/pre-authorization as required by your insurance, pre-arrange necessary tests, and allow enough time for your doctor to see you.

To Bring

Your Insurance Card, Referral Form if applicable, any medical documentation, and all your medications in their original containers. Up to 3 family members or care-givers are welcome.

To Expect

You will need to arrive 15 minutes before your appointment. If you have not had the chance to fill out the paperwork in advance, please arrive 30 minutes early to give you time to complete the forms. We will ask you to complete our Registration Form, Short Medical History, and if appropriate, Chest Pain Form. Please also review and sign our Financial Policy Form and Notice of Privacy Form. We will mail you copies of these forms to complete ahead of time, or you can print them off this web site by clicking the appropriate links.

Allow a little over an hour for your visit. The doctor will interview and examine you, and generally give you recommendations at the end of your visit.
